What is the cost associated with your services?
tango contracts directly with home health agencies, so as long as your insurance plan has no co-pay or deductible and you utilize one of our contracted providers, there is no out-of-pocket expense required for the patient. Your physician must certify a need for skilled home health services for you or your loved one, provided homebound status and a need for skilled services exist. All skilled services provided by contracted home health agencies furnish in-home services based on covered insurance guidelines.
How soon can I expect to hear from a home health agency once my physician or the discharge planner at the hospital makes a referral to tango?
Once a referral is received, tango’s referral management department will begin the search to find a local agency to render your care; one within our contracted network of quality providers. Typically, someone from the assigned agency will reach out to you within 2 business days.
If my plan of care requires a skilled nurse, what care can they provide to me in my home?
If your physician orders a skilled nurse to visit, some examples of the type of services they can provide are as follows: post-operative wound care and teaching, clinical assessments after an exacerbation of illness, such as congestive heart failure or COPD; medication reconciliation and education, ostomy care and instructions, etc. The need for skilled nursing care in the home is generally short-term.
If my physician or discharge planner includes a referral for physical therapy, what can I expect?
An order for physical therapy may be included in your plan of care if your physician has identified you might benefit from a home-based exercise or rehab program. Whether you are recovering at home after a joint replacement, or have become debilitated after a hospital stay, a physical therapist can assist in developing a home exercise program. A physical therapist can also play a role in assessing your home environment to ensure you remain safe and have the appropriate equipment on hand.
If my physician or discharge planner includes a referral for occupational therapy, what can I expect?
An occupational therapist may be included in your plan of care if your physician has identified you might benefit from a home-based program focused on restoration of fine motor movements, upper extremity rehabilitation and/or working on independence in activities of daily living, such as personal hygiene. Occupational therapy is oftentimes ordered in combination with physical therapy after medical incidents such as a stroke or trauma injury. An occupational therapist, along with physical therapy, can also assess for appropriate equipment in your home to ensure you remain safe.
If my physician or discharge planner orders speech therapy as part of my plan of care, what can I expect?
Speech therapy will be included in your plan of care if your physician has identified you might benefit from a home-based program focused on cognitive training, speech and/or swallowing disorders. A speech therapist can assist in establishing a home program to rehabilitate and/or maximize function in these areas, as well as serve as a consultant to your physician or caregiver in outlining appropriate dietary criteria to keep you safe in your home.
What if I require assistance with personal care?
Short term home health aide services focused on assisting with personal care may be requested and approved for a finite period or defined with an end date, when a skilled discipline is in the home.
What if I require social work services?
Limited social work intervention may be provided by home health agencies if a social or financial need exists for a member to remain safe in their home. To note-these services are limited and may vary depending on agency availability. Your home health provider can provide you with additional information about social work services
What if I need additional visits?
Your tango representative will coordinate directly with your home health provider to make sure you are getting the appropriate number of in-home visits.
What if I have a question about a request or utilization management?
Members are encouraged to reach out directly to their home health provider for questions regarding a plan of care. If questions cannot be resolved or are specific to tango, our staff is available weekdays between 8am-5pm, excluding holidays at 602-395-5114 or toll free at 1-877-612-7065. tango does assist members who are hearing or speech impaired with TDD/TTY services, which are available by contacting National Relay Service (711) or your local State Relay Service. Language assistance is also available.
